Create Applications

Creu / Create is aimed at young people who have mild to moderate
emotional and mental health needs. It does not cater for young people with behavioural needs or those who need additional 1 to 1 support workers.

Referrals can only be made for young people in school years 7-11.

Who can refer?

Referrals will be accepted from CAMHS, Social Services, Educational Psychology and School Based Counselling Service.

We are happy to welcome support workers to initially attend with the young people.

How to refer?

Please download the application form opposite.
Complete it with the young person to be referred and their parent/carer and send via encrypted email to: info@createproject.co.uk
