Create 2.0 Applications

 

Creu / Create 2.0 is aimed at young adults who have mild to moderate
emotional and mental health needs. It does not cater for people with behavioural needs or those who need additional 1 to 1 support workers.

Referrals can only be made for adults aged 18-25.

How to refer?

Complete it with the person to be referred.
